We are currently seeking a compassionate, organized, and team-oriented individual to join our Hospice Team as a Part-Time (Weekdays/Weekends) Licensed Vocational Nurse (LVN) in Santa Clara County, CA.

We are seeking an experienced, self-driven Part-Time (Weekdays/Weekends) Licensed Vocational Nurse (LVN) to join our Hospice team and deliver exceptional care within the local community. You will assist in implementing individualized care plans, monitor patient conditions, and provide direct nursing support in the field. You’ll work closely with our Clinical Director, Registered Nurses, and other care team members to ensure high-quality hospice care.

Implement the care plan developed by the Registered Nurse and provide treatments based on patient needs
Monitor patient conditions, report changes, and document care in a timely manner
Educate patients and caregivers on disease process, medications, and care techniques
Collaborate with other hospice team members and communicate needs to the RN or Clinical Supervisor
Ensure proper use of medical supplies and adherence to company policies
Maintain complete and accurate clinical records using Kinnser (company devices provided)
