We are searching for reliable and compassionate practical nurses to join our team. You should be able to address all the patient's concerns and make appropriate adjustments to the patients’ in-home management plan.
To be successful as a homecare nurse, you should be able to handle a wide range of tasks and possess the mental strength and resilience to care for patients and are also able to provide emotional support to family members if required.
Responsibilities
Understands and adheres to established company policies and procedures.
Implements the nursing care plan for each patient.
Provides nursing services, treatments and diagnostic and preventive procedures as assigned.
Initiates the appropriate preventative and rehabilitative nursing procedures.
Observes signs and symptoms and reports to the physician and RN, reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
Recommend referral to other personnel, those patients requiring specialized care.
Teaches and counsels the patient and family/ significant others regarding the nursing care needs and other related problems of the patient at home.
Evaluates with the RN the effectiveness of the PN's nursing service to the patient and family under the guidance of the RN.
Maintains accurate and complete records of observations, treatments, and care of patient.
Participates in medical record audit as assigned.
Attend staff meetings, patient care conferences, and in-services as scheduled.
Takes on-call duty nights, weekends, and holidays, as assigned.
Prepares clinical and progress notes.
Assist the physician and RN in performing specialized procedures.
Prepares equipment and materials for treatment.
Observes aseptic technique as required.
Assist the patient in learning appropriate self-care techniques.
